  4. Purchase orders for a minimum of $50 can be used for ordering by U.S.and
  5. Canadian corporations, agencies, or schools. To enable us to process your APDA
  6. product orders as quickly as possible, please provide the following
  7. information:

  9. WHAT TO INCLUDE IN THE PURCHASE ORDER
  10.  
  11. •  Name of a contact person and phone number
  12.  
  13. •  Tax information
  14.    Specify whether the products you are ordering are taxable or non-taxable.
  15.    If your company is tax-exempt, we must have a copy of the resale or tax
  16.    certificate on file. APDA is required to collect sales tax in every state.
  17.  
  18. •  Your APDA customer number
  19.    Your APDA customer number can be found on the mailing label. Leave blank if
  20.    it’s not readily available.
  21.  
  22. •  An authorized signature, which is required by APDA,for  approval on the
  23.    purchase order
  24.  
  25. •  Your billing address and ship-to address
  26.  
  27. •  Special note for Canadian customers
  28.     — Call 800-637-0029 for shipping costs
  29.     — Please include a completed International Letter of Assurance with your
  30.       order.

  32. ADDITIONAL INFORMATION
  33. •  The minimum amount for purchase orders is $50 per order (excluding shipping
  34.    and tax).
  35. •  Terms are net 30 days from date of invoice.
  36. •  Part numbers and prices are listed in the current APDA Tools Catalog.
  37. •  Due to the updating of some of our products, versions and prices are subject
  38.    to change without notice.
  39. •  Processing time is 48 hours. Missing required information will delay the
  40.    processing of your purchase order. If APDA does not receive a response
  41.    within 12 days of our request for the required information, the order is
  42.    subject to cancellation.
  43. •  APDA customer service representatives are available to answer your questions
  44.    Monday through Friday from 7:00 a.m. to 5:00 p.m., Pacific Standard Time.

  53. Purchase orders are accepted by mail or fax only. Orders cannot be processed
